The following information is required in your service request:
- Indicate the name(s) of all employee(s) who will be assigned the hardware/equipment.
- Where will the equipment be located? Please include building name and office number.
- Indicate what items are needed (example - laptop, Macbook, desktop, docking station, etc)
- Include how many of each item are needed? (example - 1 laptop, 2 monitors)
- What date is the equipment needed by? (Estimated shipping times are currently varied.)
